Nothing is broken. Reindex required on a source's Retrieval Accuracy tab means you changed a setting that shapes how content gets indexed — an enrichment switch, a chunking mode, a prompt, or the profile's model — so the live index no longer reflects the current settings. The old index keeps serving answers exactly as before until you act.
To clear it, open the source's Index tab and run Reindex. The new index is built with the current settings and the status returns to "Current settings match the index."
Two things worth knowing:
- Check Estimated indexing calls on the Retrieval Accuracy tab first — if you just enabled enrichment, the reindex will spend one AI call per chunk.
- If you changed settings experimentally and don't want the cost, simply change them back; the flag clears itself once settings match the index again.
The Diagnostics screen counts sources currently flagged, so you can sweep them in one sitting. See Content Enrichment & Chunking for what each setting does.
